JA NURSING WE CARE INC. Is a non-profit organization established by it's Founder Michelle Smith, on the basis of working in her Nursing field for over 15 years. It was clear to Michelle that simply focusing on the issue wasn't enough. JA Nursing We Care Inc. is an organization that addresses health and social literacy by providing clear, and reliable information, which is essential to helping people manage their health and Social needs. Health and social literacy is the ability to access, understand and act on information received. We understand, that in order to serve the community we must recognize their needs accordingly by bridging the gap between the community social sector and the health care sector.
Our MISSION is to increase the health and social literacy for vulnerable populations including youths, elderly and mental health clients through a culturally diverse lens while increasing the cultural competence of the organizations that we partner with to create this change. Our VISION is to create agents of change throughout collaborating communities by improving their health and social outcomes with appropriate referral, outreach and navigation and programming. OUR MANDATE is to enhance access to programs and services by identifying the individual needs of our clients by using a coordination of care model.This is essential when considering the social determinants that affect health and the health disparities faced with in the community. We are enthusiastically engaged in partnerships with other organizations through our outreach model and our tailored initiatives. The Process of Cultural Competence in the Delivery of Health-care and social Services is essential to changing outcomes. We view cultural competence as an ongoing process in which the health care and social service provider continuously strives to achieve the ability to effectively work within the cultural context of the client (individual, family, community). We utilize a process which involves the integration of cultural awareness, cultural sensitivity, cross cultural communication and cultural congruence in the delivery of our programs. |
